TERMS OF CONSENT TO PERSONAL DATA PROCESSING

for marketing purposes

pursuant to Act no. 101/2000 Col., on personal data protection and changes to some laws, as amended

Read these terms of consent to personal data processing carefully before you grant consent to the processing of your personal data! By providing your personal data to the Company for marketing purposes, you confirm that you have acquainted yourself with the contents of this document.

By providing your personal data to the company Galerie Teplice s.r.o., with its seat at Praha 1 - Nové Město, Washingtonova 1624/5, PSČ 110 00, the Czech Republic, Commercial Register number: 292 30 292 entered in the commercial register maintained by the Municipal court in Prague, section C, file 173327 (hereinafter referred to as the „Company“), for marketing purposes (for instance, by paying with a discount voucher distributed by the Company, by participating in a competition organised by the Company or by otherwise providing your personal data to the Company for marketing purposes) you give the Company consent to process your personal data to the extent, for the purposes and on the terms specified herein.

Scope of collected and processed Personal Data

Name and surname, title, address, telephone number and e-mail (hereinafter referred to as „Data“).

Persons to whom personal Data may be made available:

This Data may be made available to the employees of the Company.

If the Company or a part thereof is sold, merged or otherwise transferred to another entity, the Company is entitled to pass on the Data as part of the transaction. Furthermore, the Company may make part of the Data available to the authorities of public governance as well as to its external professional advisers. Moreover, the Company is entitled to make the Data accessible in cases when legal regulations directly require it to do so or in situations when such a procedure is desirable for the exercise of its rights; for the adoption of measures concerning illegal activities or to provide protection or safety to any persons.

The Company may collect and store Data in various formats (e.g. on paper, in electronic and audio formats and otherwise). The collection, storage and processing of the Data will take place in the Czech Republic. The Company has introduced safety measures that protect the confidentiality of the Data.

The purpose of the Collection and Processing of Data

Data will be used for marketing purposes (in particular the sending of newsletters, providing information about events planned by the Company and its leaseholders, offering goods and services).

The Effect of Consent

Consent is granted for a period of 10 years from the moment it is provided.

The Company shall destroy collected Data immediately after the purpose of their collection and processing activities ceases to exist, unless it is required by law to store the Data for a longer period.

You may revoke granted Consent at any time during its effect, which shall be done in writing, and such a revocation becomes effective on the day following the day such a revocation is delivered to the Company.

Voluntary provision of Data

The provision of the Data is voluntary.

Rights relating to the administration and processing of Data

In relation to Data collected and processed by the Company or any other person to which the Company transfers the Data, in compliance with the legal regulations and with this consent, you may:

• request access to your Data, i.e. you may request to be informed of the scope of the collected Data, the purpose of its collection and processing, the nature of its processing and the details of individual and judicial persons to whom the Data may be made accessible or transferred,
• request the correction of inaccuracies concerning Data, and,
• if the Company or any other person to which the Company transfers the Data processes your Data in contradiction with legal regulations, you may request and will receive an explanation and a remedy for every violation of the stipulated terms, including the blocking, correction, supplementing and destruction of certain Data.
